Included in Well Puppy Plan:
Your puppy's initial visit and all comprehensive office visits are needed until your puppy is fully vaccinated. Each visit will include full physical exams, vaccine boosters, 3 Grooming Orientation Sessions, and consultation.

All vaccines recommended by your veterinarian. These may include:

• Rabies (1)
• DAPP combination [distemper-adenovirus-parainfluenza-parvovirus] (up to 4)
• Bordetella (1)
• Lyme (2)
• Leptospirosis (2)
• Canine Influenza (2)
• One fecal screening to test for intestinal parasites and initial deworming series
• An initial dose of flea, tick, and heartworm preventative.

Plan is active from enrollment until your puppy is fully vaccinated (up to 6 months of age) per your veterinarian's recommended vaccine schedule. The puppy plan is paid at enrollment and cannot be applied retroactively. This is NOT an insurance plan.

Not Included in Well Puppy Plan:

• Additional fecal screenings and deworming medications
• Flea, tick, and heartworm preventative after the initial dose
• Spay/neuter surgeries and associated costs
• Microchipping
• Sick puppy visits
